CASE OF THE MONTH - 10/01/05

History:

A 24-year old white female roller-skater fell onto an outstretched right arm. The athlete reported the sensation of the elbow "coming out of place" then popping back in". She continued to have pain and swelling of the right elbow and reported to our clinic three days later for evaluation. The athlete reported numbness over the dorsum of the right hand following the injury, however she states this did slowly improve. She reports a past history of a right elbow dislocation in 1998 due to similar type roller-skating incident and has residual decreased extension of the elbow.

The rest of her medical history is unremarkable. She takes no medications regularly. Denies use of tobacco, EtOH, or illegal drugs. She has a good diet and notes regular normal menses.

Physical Exam:

Right Elbow: Tissue swelling without erythema over the lateral aspect of the elbow. She lacks approximately 20 degrees full extension as compared to the left elbow. She reports her baseline deficit of approximately 10 degrees of extension. Flexion is limited to 110 degrees secondary to pain. Pronation and supination are bilateral, however the patient reports pain with this maneuver. Only active range of motion testing was done. Palpation produces pain over the lateral epicondyle with less amount of pain over the radial head. Neurovascular exam was unremarkable.
